ThriveMore at Home celebrates National Home Care Month

November is National Home Care and Hospice Month, a time to honor the dedicated professionals who provide essential care and comfort to individuals in their own home. From skilled nurses and aides to social workers and volunteers, these caregivers make aging with dignity and peace possible.

ThriveMore at Home is a Continuing Care at Home program designed to help older adults age in place with confidence and support.
ThriveMore at Home empowers members to remain in their homes while receiving personalized wellness planning, preventive care, and long-term care coordination. Each member is paired with a Wellness Coordinator who helps design a proactive plan for aging, advocates during health challenges, and navigates care services when needed.
